As a Little City parent or family member, the PFG group plays a significant role in ensuring that their loved ones, as well as others, have the highest quality of life possible.
The PFG Group provides opportunities for you to network, fundraise, volunteer or simply raise awareness with other families who share a similar passion for providing the best environments, services and programs possible.


To make positive contributions to Little City for the benefit of family members and to ensure they have the opportunity to reach their fullest potential.

As a Little City parent or family member, you can play a significant and important role in providing a high quality of life for your loved one. We value your feedback as Little City is continually striving to provide the best programs and services for individuals with disabilities. We’re looking to obtain your thoughts regarding the successes and areas of improvement we need to make as an agency.

Please take a few minutes to fill out the appropriate survey related to the program your loved one participates in. All information that you provide will remain anonymous unless you otherwise specify.

Little City is seeking timely and current feedback, therefore we ask that surveys be completed at least annually.


If your family member receives services through Little City’s Center for Adult Living or Center for Employment, please fill out the following survey: LifePath Residential & Vocational Services.


If your child receives services through Little City’s ChildBridge Center for Group-Home Living or attends the ChildBridge Center for Education, please fill out the following survey: ChildBridge Residential & Center for Education.


If your loved one receives services through Little City Center for Family & Community-Based Services, please fill out the following survey: Center for Family & Community-Based Services.
