Little City's ChildBridge Center for Children and Families offers the DHS (Department of Human Services) Home-Based Waiver Program providing person-centered, in-home personal and family support services to individuals with intellectual and developmental disabilities and their families. 

Services are consistent with current best-practice models and are delivered in the home and community while increasing competency for the child in all areas of functioning.

The program empowers families providing them with the skills they need to effectively meet the needs of their family member.  If the family decides they can no longer meet the needs of their loved one at home, Service Facilitators, in conjunction with the Independent Support and Service Advocacy (ISSA) worker, assist the family and child in the transition to out-of-home living.

The Center for Children and Families' DHS Home-Based Waiver provides service to 80 families throughout the Chicagoland area.  Individuals range in ages from five to 45 years, and have a variety of intellectual disabilities, including autism, cerebral palsy, Angelman Syndrome, Batten Disease, Fetal Alcohol Syndrome, Turner Syndrome and global developmental delays.

  • Counseling and Training for Non-paid Caregivers
  • Behavior Intervention and Treatment

Geographic Service Area

The DHS Home-Based Waiver program supports families throughout Chicago with a service area that extends to Cook, DuPage and Will counties.
